ABOUT FAITH HEALTHCARE
Our History
Faith Health Care, Inc. (FHC) was founded as a non-profit organization in 2015 in response to the desperate and immediate health care needs of underserved individuals in rural Kentucky’s Wayne and Pulaski Counties.
FHC’s leadership further assess the community’s needs and, in 2019, FHC became the Community Care Clinic (CCC). Throughout 2019, CCC strategically assessed where to open a new community health center, with the goal of becoming a Federally Qualified Look-Alike Health Center.
After a year of planning, developing proper infrastructure, and constructing a new state-of-the-art health center, CCC opened its doors to serve the low-income and underserved individuals and families in the service area on December 9, 2019. The 9,900 square foot health center offers 32 medical exam rooms, a large reception and waiting area, CLIA-waived labs, and onsite phlebotomy services. Since then, Faith Healthcare has expanded to develop two additional medical clinics and six dental clinics in the Bluegrass state to continue to care for all patients, regardless of their ability to pay.

What is an FQHC?
Health centers are community-based organizations that provide high-quality primary health care to vulnerable individuals and families. They offer services like pharmacy, mental health, substance use disorder, and dental care, especially in areas where people have difficulty accessing affordable health care due to economic, geographic, or cultural barriers. By focusing on coordinated care and using modern health information technology, health centers help reduce health disparities and ensure patients with multiple health needs receive proper care.
